Женщины во время беременности могут быть особенно восприимчивы к инфекции SARSCoV-2, поскольку физиологические изменения затрагивают сердечно-сосудистую, дыхательную и иммунную системы организма. Несколько исследований указывает на то, что беременные, перенесшие COVID-19, находятся в группе высокого риска не только по развитию тяжелого течения инфекции, у них также повышена частота плаценто-ассоциированных осложнений беременности. Цель проведения литературного обзора – установить патогенетические взаимосвязи между COVID-19 и плаценто-ассоциированными осложнениями беременности. Материалы и методы исследования. Для анализа научной литературы использовались международные и российские наукометрические базы данных Google Sholar, ResearchGate, Pubmed, Elibrary. Результаты и обсуждение. Тяжелая форма COVID-19 тесно связана преэклампсией, преждевременными родами и другими неблагоприятными перинатальными исходами. Беременные женщины с инфекцией SARS-CoV-2 имеют более высокий риск материнской смертности, госпитализации в отделение интенсивной терапии, преждевременных родов и мертворождений. Однако в ряде случаев повышенная частота преэклампсии, наблюдаемая у матерей с SARS-CoV-2, может быть связана с ошибочным диагнозом и ятрогенными преждевременными родами, так как преэклампсия и COVID-19 могут иметь сходные симптомы. Вакцинация против COVID-19 рекомендуется во время беременности в группах высокого риска, чтобы предотвратить тяжелую материнскую заболеваемость и неблагоприятные исходы беременности и родов. Заключение. COVID-19 повышает риск плаценто-ассоциированных осложнений беременности. В ряде случаев COVID-19 имитирует клинику преэклампсии, поэтому требуется тщательная дифференциальная диагностика для снижения частоты ятрогенного преждевременного родоразрешения. Вакцинация у беременных является мерой профилактики инфекции COVID-19 и ее тяжелых форм и может опосредованно привести к снижению частоты больших акушерских синдромов.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Women during pregnancy may be particularly susceptible to SARS-CoV-2 infection as physiological changes affect the cardiovascular, respiratory and immune systems. Several studies suggest that pregnant women who have had COVID-19 are not only at high risk for developing a severe infection, they also have an increased incidence of placental-associated pregnancy complications. The objective of this literature review was to establish pathogenetic relationships between COVID-19 and placenta-associated pregnancy complications. Materials and methods. International and Russian scientometric databases Google Sholar, ResearchGate, Pubmed, and Elibrary were used to analyze the scientific literature. Results and discussion. Severe COVID-19 is closely associated with preeclampsia, preterm birth, and other adverse perinatal outcomes. Pregnant women with SARS-CoV-2 infection are at higher risk of: maternal death, intensive care unit hospitalization, preterm birth and stillbirth. However, in some cases, the increased incidence of preeclampsia observed in mothers with SARS-CoV-2 may be related to misdiagnosis and iatrogenic preterm birth, as preeclampsia and COVID-19 may have similar symptoms. COVID-19 vaccination is recommended during pregnancy in high-risk groups to prevent severe maternal morbidity and adverse pregnancy and childbirth outcomes. Conclusion. COVID-19 increases the risk of placental-associated pregnancy complications. In some cases, COVID-19 mimics the clinical picture of preeclampsia, so careful differential diagnosis is required to reduce the incidence of iatrogenic preterm labor.
