Фетальная макросомия ассоциируется с риском материнского и плодового травматизма, плечевой дистоции, увеличением частоты кесарева сечения, акушерских кровотечений. Прогнозирование крупных размеров плода на антенатальном этапе остается актуальным вопросом, окончательно не решенным даже при использовании современных методов медицинской визуализации. Цель исследования – оценка прогностической ценности антенатальной 2D-ультразвуковой сонографии в III триместре для определения риска рождения плода с крупными размерами. Материалы и методы. Обследовано 819 женщин, из них 365 (основная группа) родили ребенка с массой более 4000 граммов, а 454 (контрольная группа) родили доношенного ребенка в пределах гестационной весовой нормы. В сроке 32–34 недели в рамках пренатального скрининга III триместра всем женщинам проведено ультразвуковое исследование с последующим расчетом предполагаемого веса плода по формуле Хэдлока. Критериями включения послужили: антропометрические данные плода при проведении УЗИ в сроке 32–34 недели в пределах нормы гестационного возраста, т. е. в пределах коридора 10–90 процентилей. Результаты. Выявлено различие между группами: вес плодов у женщин основной группы в III триместре, рассчитанный по формуле Хэдлока, оказался достоверно выше и составил 2123 граммов (Q1; Q3 – 1948; 2278) по сравнению с контрольной группой, в которой вес составил 1990 граммов (Q1; Q3 – 1844; 2101), p &lt; 0,001. По данным ROC-анализа выявлена невысокая, но статистически значимая диагностическая ценность этого параметра для прогноза фетальной макросомии. Обсуждение. Практическую значимость определения риска фетальной макросомии трудно переоценить. В результате использования прогностических способов возможно принять меры для предотвращения материнского и плодового травматизма, профилактики перинатальной заболеваемости. Результаты нашего наблюдения показывают, что даже изолированное использование оценки предполагаемого веса плода по методу Хэдлока, произведенное в условиях учреждения III уровня, позволяет выделить группу беременных с угрозой по реализации риска фетальной макросомии, с приемлемыми чувствительностью и специфичностью. При этом пороговым показателем веса плода в сроке 32–34 недели является значение 2032 грамма. Очевидно, что ультразвуковое исследование в 32–34 недели остается необходимым в качестве селективной скрининговой технологии, например при наличии известных факторов риска фетальной макросомии. Заключение. Антенатальная 2D-ультразвуковая сонография для определения риска рождения плода с крупными размерами, проведенная в рамках пренатального скрининга III триместра, является инструментом селективного скрининга. Разработка интегральных многофакторных прогностических моделей фетальной макросомии, основанных на подсчете предполагаемого веса плода по способу Хэдлока в сроке 32–34 недели, представляется перспективным направлением, которое позволит существенно улучшить качество перинатальной помощи.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Fetal macrosomy is associated with the risk of maternal and fetal trauma, shoulder dystocia, increase frequency of cesarean section, and obstetric hemorrhage. Prediction of large fetal size at the antenatal stage remains an urgent issue, definitively unresolved even with the use of modern medical imaging techniques. Materials and Methods. A total of 819 women were examined, of whom 365 (the study group) gave birth to a baby weighing more than 4000 grams, and 454 (the control group) gave birth to a full-term baby within the gestational weight norm. At 32–34 weeks of gestational screening in the third trimester, all women underwent an ultrasound examination followed by calculation of estimated fetal weight using the Hadlock formula. The inclusion criteria were fetal anthropometric data at 32–34 weeks' gestational age within the normal range, i.e. within the 10-90th percentile corridor. Results. There was a difference between the groups, the fetal weight in women in the main group in the third trimester, calculated by the Hadlock formula, was significantly higher and was 2123 grams (Q1; Q3 – 1948; 2278) compared to the control group, in which the weight was 1990 grams (Q1; Q3 – 1844; 2101), p &lt; 0.001. ROC-analysis revealed a low, but statistically significant diagnostic value of this parameter for the prognosis of fetal macrosomia. Discussion. The practical importance of determining the risk of fetal macrosomia cannot be overestimated. Using prognostic methods, it is possible to take measures to prevent maternal and fetal trauma and to prevent perinatal morbidity. Our results show that even the isolated use of the estimated fetal weight estimation by the Hadlock method, performed in a Level III institution, allows us to identify a group of pregnant women at risk for fetal macrosomia with acceptable sensitivity and specificity. The threshold fetal weight at 32–34 weeks is 2032 grams. Obviously, ultrasound at 32–34 weeks remains essential as a selective screening technique, e.g. in the presence of known risk factors for fetal macrosomia. Conclusion. Antenatal 2D ultrasound sonography to determine the risk of fetal oversize fetuses performed as part of third-trimester prenatal screening is a tool for selective screening.
