В связи с изменением возрастной структуры населения земного шара в будущем следует ожидать увеличение числа гериатрических пациентов, которым будет проводиться эндопротезирование крупных суставов. Возрастная слабость и существующие сопутствующие заболевания создают проблемы для всего лечебного процесса.</p><p>Цель исследования − определить эффективность применения комплексного гериатрического ведения пациентов пожилого и старческого возраста до и после оперативных вмешательств по эндопротезированию тазобедренного и коленного суставов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен анализ публикаций использования комплексной гериатрической оценки при эндопротезировании крупных суставов у больных пожилого и старческого возраста с остеоартрозом. Поиск осуществлен в электронных базах данных и библиотеках Web of Science, Coсhrane library, PubMed, eLibrary по поисковым словам: comprehensive geriatric assessment, комплексная гериатрическая оценка.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Наблюдается учащение случаев эндопротезирования крупных суставов у ортогериатрических больных с остеоартрозом. Для данной группы пациентов важным вопросом является минимальная травматичность от анестезии и эндопротезирования, а также обеспечение немедленной послеоперационной мобилизации больного. Необходимым организационным элементом лечения является индивидуальный план подготовки пациента к оперативному вмешательству и послеоперационному ведению, направленный на предотвращение развития преходящих когнитивных нарушений и других осложнений, связанных с гериатрическим статусом, в частности падений.</p></sec><sec><title>Заключение</title><p>Заключение. Комплексная гериатрическая оценка играет важнейшую роль в периоперационном введении гериатрических больных при эндопротезировании крупных суставов. Своевременная идентификация гериатрических больных на предоперационном этапе устраняет факторы риска.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Due to the changing age structure of the world’s population, an increase in the number of geriatric patients undergoing endoprosthetics of large joints is to be expected in the future. Age-related frailty and existing comorbidities pose problems for the entire treatment team.</p><p>The aim of the study was to determine the effectiveness of comprehensive geriatric management of elderly and senile patients before and after endoprosthetic of the hip and knee joints.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An analysis of publications on the use of comprehensive in endoprosthetic of large joints in elderly and senile patients with osteoarthritis was performed. Searched in the electronic databases and libraries Web of Science, Conchrane library, PubMed, eLibrary using the search words: comprehensive geriatric assessment.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. There is an increasing incidence of large joints endoprosthetic in orthogeriatric patients with osteoarthritis. Anaesthesia and arthroplasty in this group of patients is an important issue, as well as ensuring immediate postoperative mobilization of the patient. An individualized patient preparation plan for surgical intervention and postoperative management, aimed at preventing the development of transient cognitive impairment and other complications associated with geriatric status, in particular falls, is a necessary organizational element of treatment.</p></sec><sec><title>Conclusion</title><p>Conclusion. A comprehensive geriatric assessment plays a crucial role in the perioperative introduction of geriatric patients for endoprosthetic of large joints.
