Преждевременная отслойка нормально расположенной плаценты (ПОНРП) продолжает занимать одну из ведущих причин кровотечений во время беременности и родов, представляющих непосредственную угрозу здоровью и жизни как матери, так и плоду, внося весомый вклад в структуру материнской и неонатальной заболеваемости и смертности. Изучение предикторов и прогнозирование тяжести последствий этого осложнения является одним из направлений научного поиска.</p><p>Цель работы — расширить возможности прогнозирования массивного кровотечения, являющегося следствием преждевременной отслойки плаценты, на основе анализа дородовых лабораторных показателей системы гемостаза в качестве универсального инструмента для оптимизации выбора лечебной тактики.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведена ретроспективная оценка взаимосвязи объема кровотечения и дородовых лабораторных показателей системы гемостаза у 46 пациенток с преждевременной отслойкой плаценты, родоразрешенных путем кесарева сечения в 2020 г. на базе Областного перинатального центра Областной детской клинической больницы, с использованием методов статистического анализа количественных показателей.</p></sec><sec><title>Результаты</title><p>Результаты. При сопоставлении объема кровопотери с дородовыми показателями гемостаза выявлена прогностическая значимость и построены уравнения парной линейной регрессии для следующих параметров: уровень тромбоцитов; тромбокрит; протромбиновое время; протромбин по Квику; международное нормализованное отношение; активированное частичное тромбопластиновое время. Не было выявлено взаимосвязи между объемом кровопотери и средним объемом тромбоцитов (MPV) (р = 0,231), шириной распределения тромбоцитов по объему (PDW) (р = 0,121), содержанием крупных тромбоцитов в крови (P-LCR) (р = 0,205), тромбиновым временем (р = 0,79), значением фибриногена (р = 0,07).</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Определение тесноты связи между исследуемыми параметрами и объемом кровопотери при преждевременной отслойке плаценты и вычисление формул парной регрессии с доверительным интервалом позволяет нам говорить о высокой прогностической ценности этих показателей.</p></sec><sec><title>Заключение</title><p>Заключение. Таким образом, на основании полученных результатов расширены возможности прогнозирования объема акушерского кровотечения, обусловленного преждевременной отслойкой нормально расположенной плаценты, для выбора оптимальной и своевременной лечебной тактики. Результаты исследования могут быть использованы в построении алгоритма принятия решений и врачебной тактики мультидисциплинарной бригады во время оказания помощи пациентке с преждевременной отслойкой плаценты, однако требуется дальнейшее проведение исследований в рассматриваемом направлении для построения универсальной формулы прогноза объема кровотечения при ПОНРП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Placental abruption continues to be one of the leading causes of bleeding during pregnancy and delivery, posing a direct threat to the health and life of both mother and fetus, making a significant contribution to the structure of maternal and neonatal morbidity and mortality. The study of predictors and prediction of the severity of the consequences of this complication is one of the areas of scientific research.</p><p>The purpose of the study is to expand the possibilities of predicting massive bleeding, which is a consequence of premature placental abruption, based on the analysis of prenatal laboratory parameters of the hemostasis system as a universal tool for optimizing the choice of treatment tactics.</p></sec><sec><title>Material and methods</title><p>Material and methods. A retrospective assessment of the relationship between the volume of bleeding and prenatal laboratory parameters of the hemostasis system was carried out in 46 patients with premature detachment of the placenta delivered by caesarean section in Sverdlovsk Regional Children Clinical Hospital, Ekaterinburg in 2020 using the methods of statistical analysis of quantitative indicators.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. When comparing the volume of blood loss with prenatal indicators of hemostasis, prognostic significance was revealed and paired linear regression equations were constructed for the following parameters: platelet level, plateletcrit, prothrombin time, Quick prothrombin time, activated partial thromboplastin time. And no relationship was found between the volume of blood loss and MPV (p = 0.231), PDW (p = 0.121), P-LCR (p = 0.205), thrombin time (p = 0.79), fibrinogen value (p = 0.07).</p></sec><sec><title>Conclusion</title><p>Conclusion.
