Острый постманипуляционный панкреатит (ОПМП) — частое осложнение после вмешательств на большом дуоденальном сосочке двенадцатиперстной кишки. Частота развития ОПМП может достигать 15 %, из них тяжелым течением характеризуется 5–10 % случаев.</p><p>Цель работы — определение возможности коррекции гемокоагуляционных нарушений и снижения выраженности синдрома эндогенной токсемии при тяжелом течении ОПМП.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Выполнено сравнительное исследование выраженности гемокоагуляционных нарушений и токсемии на 40 животных (нелинейные крысы-альбиносы) с тяжелым течением острого панкреатита на экспериментальной модели ОПМП. В целях уменьшения выраженности гемокоагуляционных нарушений и уровня токсемии в исследовании использовано соединение L-17 из новой группы органических соединений замещенных тиадиазинов в суточной дозе 40 мг/кг.</p></sec><sec><title>Результаты</title><p>Результаты. Показано развитие гиперкоагуляции (повышение концентрации D-димеров (это правильное обозначение) и изменение показателей тромбоэластограммы) и выраженной токсемии (повышение концентрации веществ средней инизкой молекулярной массы (ВНСММ) и коэффициента распределения К (соотношение ВНСММ плазмы и эритроцитов), снижение связывающей способности альбумина (ССА)) у животных с экспериментальной моделью ОПМП. Введение крысам с ОПМП соединения L-17 приводит к нормализации показателей тромбоэластограммы, снижению концентрации D-димеров, а также уменьшению уровня токсемии (снижение концентрации ВНСММ и коэффициента К, повышение ССА).</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Течение ОПМП в эксперименте сопровождается развитием тяжелых гемокоагуляционных нарушений и выраженной токсемией. Эффективными показателями, отражающими тяжесть течения ОПМП, являются D-димеры (гемокоагуляция) исвязывающая способность альбумина (токсемия).</p></sec><sec><title>Заключение</title><p>Заключение. Применение соединения L-17 из группы тиадиазинов при экспериментальном ОПМП тяжелой степени снижает выраженность гемокоагуляционных нарушений и уменьшает тяжесть течения полиорганной дисфункции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Acute post-manipulation pancreatitis (APMP) is a frequent complication aﬅer interventions on the major duodenal papilla. The APMP incidence reaches 15 %, of which 5–10 % are severe. The goal of the research is to determine the possibility of correcting hemocoagulation disorders and reducing endogenous toxemia syndrome severity in APMP.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A comparative study was performed in 40 animals (nonlinear albino rats) with severe acute pancreatitis on APMP experimental model. L-17 compound from a new group of organic compounds substituted thiadiazines at a daily dose of 40 mg/kg was used.</p></sec><sec><title>Results</title><p>Results. The development of hemocoagulation disorders and severe toxemia in animals with an experimental model of APMP has been shown. The indicators of the severity of hemocoagulation disorders and toxemia were studied in experimental APMP against the background of the introduction of the compound L-17 from the group of substituted thiadiazines.</p></sec><sec><title>Discussion</title><p>Discussion. The course of APMP in the experiment is accompanied by the development of severe hemocoagulation disorders and severe toxemia. Eﬀective indicators reﬂecting the severity of APLP are D-dimers (hemocoagulation) and albumin binding capacity (toxemia).</p></sec><sec><title>Conclusion</title><p>Conclusion. The course of experimental severe APMP is accompanied by pronounced hemocoagulation disorders development involved in the pathogenesis and progression of multiple organ dysfunction, that is characterized by a high level of endogenous toxemia.
