Большинство сосудистых заболеваний сетчатки связано с системными факторами риска, такими как гипертония, сахарный диабет, возраст и курение. Однако при их отсутствии требуется дообследование, необходимое для выявления возможных гематологических нарушений.</p><p>Цель работы — описать клинический случай окклюзии центральной вены сетчатки (ОЦВС) как дебюта хронического миелопролиферативного новообразования.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В рамках исследования проведены молекулярно-генетический анализ на выявление драйверной мутации JAK2V617F в периферической крови в качестве скрининговой диагностики Ph-негативных хронических миелопролиферативных новообразований (ХМПН), цитологическое и цитогенетическое исследования аспирата костного мозга, а также морфологическое исследование трепанобиоптата костного мозга.</p></sec><sec><title>Результаты</title><p>Результаты. У пациента, госпитализированного в офтальмологическое отделение с диагнозом ОЦВС правого глаза, определен мутантный аллель гена JAK2V617F с диагностически значимой аллельной нагрузкой 14,61 %. Пациент направлен для дальнейшего обследования в Московский городской гематологический центр (МГГЦ) Городской клинической больницы имени С. П. Боткина — установлен диагноз «истинная полицитемия».</p></sec><sec><title>Обсуждение</title><p>Обсуждение. ХМПН — группа опухолевых заболеваний кроветворной ткани, характеризующаяся злокачественной трансформацией стволовых клеток костного мозга с последующей их клональной пролиферацией. Значимую роль в патогенезе хронических ХМПН играют цитогенетические и молекулярно-генетические нарушения. Истинную полицитемию, эссенциальную тромбоцитемию, первичный миелофиброз относят к группе классических Ph-негативных ХМПН. Тромботические, сосудистые и геморрагические осложнения являются наиболее распространенными причинами смертности у пациентов с такой патологией.</p></sec><sec><title>Заключение</title><p>Заключение. Ранее были описаны различные офтальмологические проявления и симптомы у пациентов с ХМПН. Наблюдаемые осложнения разнообразны и часто являются вторичными по отношению к патогномоничным для этих заболеваний гематологическим нарушениям. Стоит рассмотреть клиническую ценность молекулярно-генетического анализа для выявления мутации JAK2V617F у пациентов с ОЦВС для ранней диагностики ХМПН и своевременного назначения соответствующей терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Most retinal vascular diseases are associated with systemic risk factors such as hypertension, diabetes mellitus, age and smoking. However, in their absence, further examination is required to identify possible hematological disorders.</p></sec><sec><title>Purpose</title><p>Purpose. Improve the diagnostic and management scheme for patients with occlusion of the central retinal vein or its branches.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A clinical case of the onset of polycythemia vera with the central retinal vein occlusion is described. As part of the study, a molecular genetic analysis was carried out to identify the driver mutation JAK2V617F in peripheral blood as a screening diagnosis of Ph-negative chronic myeloproliferative neoplasms (CMN), a cytological, cytogenetic study of bone marrow aspirate and a morphological study of bone marrow trephine biopsy.</p></sec><sec><title>Results</title><p>Results. A mutant allele of the JAK2V617F gene with a diagnostically signiﬁcant allele load of 14.61 % was determined in a patient hospitalized in the ophthalmology department with a diagnosis of central retinal vein occlusion (CRVO) of the right eye. The patient was referred for further examination to the Moscow City Hematology Center of Botkin City Clinical Hospital, where, aﬅer examination, the diagnosis was established: True polycythemia.</p></sec><sec><title>Discussion</title><p>Discussion. CMN are a group of tumor diseases of hematopoietic tissue characterized by malignant transformation of bone marrow stem cells with their subsequent clonal proliferation. Cytogenetic and molecular genetic disorders play a signiﬁcant role in the pathogenesis of chronic CMN. True polycythemia, essential thrombocytemia, primary myeloﬁbrosis belong to the group of classical Ph-negative CMP, Thrombotic, vascular and hemorrhagic complications are the most common causes of death in patients with this pathology.</p></sec><sec><title>Conclusion</title><p>Conclusion. Previously, various ophthalmological manifestations and symptoms have been described in patients with CMN. The observed complications are diverse and oﬅen secondary to the pathognomonic hematological disorders for these diseases.
