Гипертензивные расстройства в период беременности, несмотря на множество эпидемиологических исследований, научных разработок, совершенствование лабораторно-инструментальных методик для поиска предикторов преэклампсии, являются серьезной проблемой перинатального акушерства. Одним из таких направлений может стать углубленное офтальмологическое исследование.</p><p>Цель работы — изучить связи офтальмологических нарушений и лабораторно-клинических показателей у женщин с гипертензивными осложнениями беременности.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Выполнен ретроспективный анализ 573 случаев гипертензивных осложнений беременности. Возраст пациенток варьировал от 15 до 50 лет. Все женщины прошли стандартное общеклиническое и акушерское обследования, необходимый комплекс лабораторных и инструментальных методов исследования, обследованы терапевтом и офтальмологом. Проведен корреляционный анализ некоторых лабораторно-клинических признаков артериальной гипертензии (АГ) и преэклампсии (ПЭ), в т. ч. офтальмологических признаков ПЭ.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что признаки ангиопатии, отека макулы и перипапиллярного отека находятся в корреляции с лабораторно-клиническими признаками гипертензивных осложнений беременности. ПЭ сопровождалась явлениями ангиопатии сетчатки в 67 % случаев, определен тренд на уменьшение диаметра артерий с увеличением степени тяжести ПЭ. В 9 случаях (1,86 %) при ПЭ выявлен перипапиллярный отек. При его наличии наблюдались более ранние сроки родов и низкая оценка по Апгар у новорожденного, чем при отсутствии отека. Отек макулы зарегистрирован в 3 случаях (0,62 %) на фоне гестационного сахарного диабета.</p></sec><sec><title>Заключение</title><p>Заключение. Сравнительный анализ показал наличие корреляционных связей между признаками гипертензивных осложнений беременности и изменениями органа зрения. Расширение офтальмологических исследований может дать новые маркеры-индикаторы для дифференциальной диагностики гипертензивных осложнений беременности, оценки тяжести состояния, динамического наблюдения и выбора тактики ведения пациенток с гипертензивными осложнениями беременности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Hypertensive disorders during pregnancy, despite many epidemiological studies, scientific developments, and the improvement of laboratory and instrumental techniques for searching for predictors of preeclampsia, are a serious problem of perinatal obstetrics. One of these areas may be an in-depth ophthalmological examination.</p><p>The aim of the work is to perform an analysis of ophthalmological disorders and laboratory, clinical parameters in women with hypertensive complications of pregnancy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective analysis of 573 cases of hypertensive pregnancy complications was performed. The age of the patients ranged from 15 to 50 years. All patients received a standard general clinical and obstetric examination, examined by an ophthalmologist. A correlation analysis of some laboratory and clinical signs of arterial hypertension (AH) and preeclampsia (PE), including ophthalmological signs of PE, was carried out. Results: PE was accompanied by retinal angiopathy in 67 % of cases. In 9 cases (1.86 %), peripapillary edema was detected in PE. In the presence of peripapillary edema, earlier delivery dates and a lower Apgar score were observed in the newborn. Macular edema was registered in 3 cases (0.62 %) against the background of gestational diabetes mellitus.</p></sec><sec><title>Conclusion</title><p>Conclusion. Comparative analysis has shown the presence of correlations between signs of hypertensive complications of pregnancy and changes in the organ of vision.
