Несмотря на успехи в ранней диагностике, в 5–10 % случаев меланома выявляется на IV стадии. В последние годы произошел прорыв в лекарственном лечении метастатической меланомы. На этом фоне современные данные по общей выживаемости (ОВ) этой группы больных в России актуальны, но публикуются редко.</p><p>Цель работы — оценка ОВ больных меланомой IV стадии в зависимости от клинических и биологических характеристик опухоли, проведенного лечения в условиях реальной клинической практики.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Ретроспективно изучена медицинская документация в медицинской информационной системе «Медофис» и региональной информационной системе ОНКОР 300 пациентов, жителей Свердловской области, с впервые установленной IV стадией меланомы кожи, впервые выявленной с 1 января 2014 г. по 1 января 2023 г. Наблюдение осуществлялось до 1 февраля 2024 г.</p></sec><sec><title>Результаты</title><p>Результаты. Противоопухолевую лекарственную терапию по поводу злокачественной меланомы кожи получили 61,3 % пациентов. Анализ на BRAF-мутацию выполнен у 46,3 % от общего числа пациентов (75,5 % от получивших специализированное лечение). Мутация в гене BRAF выявлена в 52,5 % случаев. 1-летняя ОВ больных злокачественной меланомой кожи составила 47,0 %, 3-летняя — 27,3 %. Показатели ОВ были лучше в группе больных с метастазами в ЦНС и лимфатические узлы, хуже — с метастазами в печень. 1-и 3-летняя ОВ пациентов, получавших таргетную терапию, — 66,7 % и 38,9 % соответственно. Эти же показатели для иммунной терапии составили 80,8 % и 53,4 %. С 2014 по 2023 г. удельный вес пациентов, получавших таргетную терапию или терапию ингибиторами контрольных точек, вырос с 11,5 % до 69,7 %.</p></sec><sec><title>Заключение</title><p>Заключение. Отмечен рост медианы ОВ, 1-и 3-летней ОВ в зависимости от времени диагностирования заболевания, проведения специализированного лечения, количества линий лекарственной терапии, применения таргетных препаратов, ингибиторов контрольных точек.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Despite advances in early diagnosis, in 5–10 % of cases, melanoma is detected at stage IV. In recent years, there have been breakthroughs in the drug treatment of metastatic melanoma. Against this background, modern data on overall survival (OS) of this group of patients in Russia are relevant, but rarely published.</p><p>Objective — to assess the OS of patients with stage IV melanoma depending on the clinical and biological characteristics of the tumor and the treatment performed in real clinical practice.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We retrospectively studied medical documentation in the Medofis medical information system and the regional information system ONKOR of 300 patients, residents of the Sverdlovsk region, with the first stage IV of skin melanoma, first identified from 1 January 2014 to 1 January 2023. Observation was carried out until 1 February 2024.</p></sec><sec><title>Results</title><p>Results. 61.3 % of patients received antitumor drug therapy. Analysis for the BRAF mutation was performed in 46.3 % of the total number of patients (75.5 % of those who received specialized treatment). A mutation in the BRAF gene was detected in 52.5 % of cases. The 1‑year OS of patients with malignant melanoma of the skin was 47.0 %, the 3‑year OS was 27.3 %. Survival rates for patients receiving targeted or immune therapy were higher. From 2014 to 2023, the proportion of patients receiving targeted therapy or checkpoint inhibitor therapy increased from 11.5 % to 69.7 %.</p></sec><sec><title>Conclusions</title><p>Conclusions.
