Число лиц с тяжелыми многооскольчатыми переломами дистального отдела плечевой кости увеличивается ежегодно. Эта тенденция негативно сказывается на исходах лечения, что подтверждает актуальность проблемы оказания помощи пациентам с внутрисуставными переломами дистального отдела плеча (ВПДОП).</p><p>Целью исследования является оценка эффективности хирургического лечения пациентов с ВПДОП посредством нового комплексного подхода к лечению этого типа повреждений.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы результаты лечения 68 женщин и 149 мужчин с ВПДОП за 2001–2022 гг.: в контрольной группе представлены ретроспективные данные 137 случаев; основной — 80 прецендентов лечения с использованием инновационных разработок.</p></sec><sec><title>Результаты</title><p>Результаты. В клиническую практику внедрены инновационные разработки в хирургии локтевого сустава (ЛС), включающие в себя новое устройство для репозиции фрагментов кости, компоновку аппарата внешней фиксации ЛС и метод трансолекранного доступа. Эти разработки позволили повысить результативность лечения и снизить количество осложнений при выполнении чрескостного остеосинтеза с 16,67 % до 5,56 %.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. К факторам, влияющим на послеоперационную функциональность ЛС, относятся сложность перелома, выбор техники фиксации и хирургического доступа. Для снижения риска осложнений разработан комплексный подход, включающий в себя оптимизацию диагностики, лечения и реабилитации пациентов. Метод позволяет избежать тотального эндопротезирования ЛС, обеспечить клинические результаты без ограничения активности и анатомическое восстановление суставной поверхности и функций ЛС.</p></sec><sec><title>Заключение</title><p>Заключение. Использование усовершенствованных методов оптимизации хирургического лечения пациентов с ВПДОП позволяет снизить количество осложнений. Открытые методы ассоциированы с высоким риском осложнений, что требует их ограниченного применения. Разработанные методики рекомендованы для включения в клинические протоколы, особенно при сложных многоплоскостных переломах.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The number of people with severe multicomminuted fractures of the distal humerus is increasing every year. This trend has a negative impact on the outcomes of treatment of such fractures, which confirms the urgency of the problem of providing care to patients with intra-articular fractures of the distal shoulder.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The results of treatment of 217 patients, 2001–2022: the control group was retrospective data of 137 cases with fractures of the humerus, the main group was 80 patients treated using innovative developments.</p></sec><sec><title>Results</title><p>Results. Innovative developments in elbow joint surgery, including a new device for bone fragment reposition, an external fixation device for elbow joint, and a transosseous access method, have been introduced into clinical practice. These developments have improved the treatment outcome and reduced the number of complications during transosseous osteosynthesis from 16.67 % to 5.56 %.</p></sec><sec><title>Discussion</title><p>Discussion. Factors affecting postoperative elbow joint functionality include fracture complexity, choice of fixation technique, and choice of surgical approach. The method allows to avoid total elbow joint endoprosthetics, providing clinical results without significant limitation of activity and to ensure anatomical restoration of the articular surface and functions. To reduce the risk of complications, an integrated approach has been developed, including optimization of diagnostics, treatment, and rehabilitation.</p></sec><sec><title>Conclusion</title><p>Conclusion.
