Введение. Баллоноклеточный невус (БН) диагностируется крайне редко, когда на долю баллонных клеток приходится более 50 % новообразования и отсутствуют явления клеточного атипизма.

Цель работы — представить морфологическую характеристику крайне редкой патологии кожи (БН) у ребенка 5 лет в необычной локализации (перианальная область), определяющую дальнейшую тактику ведения пациента.

Описание клинического случая. У 5-летнего мальчика в перианальной области обнаружено безболезненное округлое образование желтоватого цвета до 0,8 см. Дерматоскопия: образование с гладкой и ровной поверхностью, желтоватым окрашиванием центральных отделов и легким коричневатым оттенком периферии. При надавливании дерматоскопа — эластичность, деформация и смещаемость образования вместе с контактной платой. Под общей анестезией образование иссечено. Макроскопически: форма образования округлая, вид симметричный, консистенция мягко-эластическая. Микроскопически: гнезда меланоцитов на границе эпидермиса и дермы. При распространении в дерму меланоциты трансформируются в крупные светлые баллонные клетки, на долю которых приходится до 90 % опухоли. Полиморфизм клеток, ядерная атипия, митозы отсутствуют. Иммуногистохимически: экспрессия Melan-A, S100, HMB-45 баллонными клетками, что указывает на их принадлежность к продуцирующим меланин. Экспрессия panCK, CD68 и СD10 отсутствует. Диагностирован БН кожи перианальной области, ассоциированный с пограничными меланоцитарными невусами кожи этой зоны.

Обсуждение

Обсуждение. Проведено обсуждение полученных результатов с литературными данными о частоте регистрации, возрасте пациентов, локализации патологического процесса и дерматоскопических особенностях БН. Подробно обсуждены морфологические и иммуногистохимические характеристики невуса с выделением иммуногистохимических маркеров такой опухоли. Приведены литературные данные о предполагаемом генезе БН.

Заключение

Заключение. Дифференциальная диагностика и постановка правильного диагноза стали возможными при совместной работе дерматолога, хирурга, патологоанатома и сопоставлении результатов осмотра пациента, дерматоскопии, гистологических и иммуногистохимических данных. Морфологический диагноз в приведенном наблюдении является чрезвычайно важным, поскольку определяет вектор дальнейших исследований и тактику ведения пациентов.

Introduction

Introduction. A rare observation of balloon cell nevus (BN) in a 5-year-old child in the skin of the perianal region is given.

The purpose of this clinical case report is to present the morphological characteristics of an extremely rare skin pathology (BN) in a 5-year-old child in an unusual location (the perianal area) which determines the patient’s subsequent management.

Description of the clinical case. A rounded yellowish formation up to 0.8 cm was found in the perianal region. A formation with a smooth surface with a brownish tint of the periphery, elastic and displaceable together with the dermatoscope board. Under general anesthesia, a rounded formation of a symmetrical appearance, soft-elastic consistency was excised. Microscopically: nests of melanocytes at the border of the epidermis and dermis with transformation into large light balloon cells (up to 90 % of the tumor). Cell polymorphism, nuclear atypia, mitoses are absent. Immunohistochemically: positive expression of Melan-A, S100, HMB-45 by balloon cells, negative expression of panCK, CD68 and CD10.

Discussion of the clinical case. Morphological and immunohistochemical characteristics of the nevus with isolation of immunohistochemical markers of this tumor are discussed in detail. Literature data on the alleged genesis of BN are presented.

Conclusion

Conclusion. The morphological diagnosis in the above observation is extremely important, since it determines the vector of further studies and the tactics of patient management.
