Введение. Значительное повышение качества и увеличение продолжительности жизни больных, инфицированных вирусом иммунодефицита человека (ВИЧ) на фоне лечения антиретровирусными препаратами (АРВП), обусловливает потребность в реализации репродуктивной функции этих пациентов. Участие суррогатной матери в лечении бесплодия у ВИЧ-инфицированных потенциальных родителей, предусмотренное российским законодательством, требует дополнительных решений ряда клинических и организационных проблем.

Цель работы

Цель работы: на примере конкретной клинической ситуации с учетом литературных данных обосновать программу суррогатного материнства в качестве метода выбора для лечения бесплодия у пациентов с ВИЧ-позитивным статусом и коморбидным онкологическим заболеванием, а также междисциплинарный подход к организации его проведения.

Материалы

Материалы. Сообщение касается случая применения программы суррогатного материнства для супружеской пары с позитивным ВИЧ-статусом в 4-й стадии заболевания с использованием спермы мужа и донорских ооцитов. Применение донорских ооцитов в данной ситуации обусловлено наступившей менопаузой на фоне коморбидного тяжелого онкозаболевания, потребовавшего многократных курсов химиотерапии.

Заключение

Заключение. Потенциальные биологические родители с положительным ВИЧ-статусом в 4 стадии заболевания могут иметь коморбидную онкологическую патологию, которая сама по себе, а также ее лечение способствуют снижению фертильности, а для будущей биологической мамы являются препятствием для самостоятельного вынашивания беременности. В связи с этим программа суррогатного материнства является методом выбора для лечения бесплодия в данной клинической ситуации. Информирование суррогатной мамы о потенциальных рисках инфицирования при участии генетических родителей с ВИЧ-позитивным статусом на различных этапах ЭКО и о профилактических мероприятиях в период вынашивания беременности целесообразно проводить акушерам-гинекологам (репродуктологам) совместно с инфекционистом областных центров по профилактике и борьбе со СПИДом.

Introduction

Introduction. A significant increase in the quality and increase in the life expectancy of HIV patients during treatment with antiretroviral drugs necessitates the implementation of the reproductive function in these patients. The participation of a surrogate mother in the treatment of infertility in HIV-infected potential parents, vided by the Russian legislation, requires additional solutions to a number of clinical and organizational problems. The aim was to substantiate a surrogacy program as a method of choice for the treatment of infertility in patients with HIV-positive status and comorbid cancer and also a multidisciplinary approach to organizing it on the example of a specific clinical situation and based on the analysis literature data.

Materials

Materials. We report a case of using a surrogacy program for a married couple with a positive HIV status in the 4th stage of the disease using the husband's sperm and donor oocytes. The use of donor oocytes in this situation is due to the onset of menopause against the background of a comorbid severe cancer that required multiple courses of chemotherapy.

Conclusion

Conclusion. Potential biological parents with a positive HIV status in stage 4 of the disease may have a comorbid oncological pathology, which in itself, as well as its treatment, contributes to a decrease in fertility, and for the future biological mother, it is an obstacle to independently carrying a pregnancy. In this regard, the surrogacy program is the method of choice for the treatment of infertility in this clinical situation. It is advisable for obstetricians-gynecologists (reproductologists) to inform the surrogate mother about the potential risks of infection with the participation of genetic parents with HIV-positive status at various stages of IVF and about preventive measures during gestation in conjunction with the infectious disease specialist of the regional centers for the prevention and control of AIDS.
