Выбор рационального метода лечения при раке вульвы является одной из наиболее актуальных и трудных проблем современной клинической онкологии. В большинстве случаев рак вульвы возникает у лиц пожилого и старческого возраста, а также части пациенток в местнораспространенной форме.</p><p> Цель исследования — провести оценку возможности реконструкции после хирургического лечения рака вульвы.</p></sec><sec><title>Материалы и методы</title><p> Материалы и методы. Проведено исследование, включившее 151 пациентку с плоскоклеточным раком вульвы, которым в онкологическом гинекологическом отделении Челябинского областного клинического центра онкологии и ядерной медицины проведено хирургическое лечение по различным методикам в объеме: расширенная вульвэктомия с резекцией рядом расположенных анатомических структур с реконструктивно-пластическим компонентом.</p></sec><sec><title>Результаты</title><p>Результаты. Проведена оценка оригинальных способов пластики послеоперационной раны после вульвэктомии, выявлен минимальный риск осложнений. Представленные технологии дают возможность использования дополнительных вариантов реконструкции раневого дефекта и имеют ряд преимуществ по сравнению с применяемой ранее пластикой двумя кожно-фасциальными лоскутами с медиальных поверхностей бедер. Дискуссия. Исследования показали, что реконструкция вульвы с использованием кожных лоскутов может избежать осложнений и улучшить качество жизни пациентов. Реконструкция лоскутами на данный момент не является общепринятым стандартом лечения рака вульвы.</p></sec><sec><title>Заключение</title><p> Заключение. Методы закрытия раневого дефекта возможны после радикальной вульвэктомии у пациенток с плоскоклеточным раком вульвы и резекцией смежных анатомических структур без сокращения объема хирургического лечения. Данные способы закрытия раневого дефекта способствуют снижению  послеоперационных осложнений и значительному уменьшению послеоперационного пребывания в лечебном учреждении.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The choice of a rational method of treatment in vulvar cancer is one of the most actual and difficult problems of modern clinical oncology. In the majority of cases vulvar cancer occurs in elderly and senile patients, as well as in some patients with locally advanced form.</p><p> The aim of the investigation was to assess the possibility of reconstruction after surgical treatment of vulvar cancer.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. A study was conducted involving 151 patients with squamous cell vulvar cancer in whom the gynecological oncology department of Chelyabinsk regional clinical center of oncology and nuclear medicine performed surgical treatment by various methods in the following volume: dilated vulvectomy with the resection of adjacent anatomical structures with reconstructive and plastic component.</p></sec><sec><title>Results</title><p>Results. The original ways of plasty of the postoperative wound after vulvectomy were evaluated, the minimum risk of  complications was revealed. The presented technologies permit to use additional variants of the wound defect reconstruction and have a number of advantages in comparison with two dermalfascial flaps from the medial surfaces of the femur used earlier.</p></sec><sec><title>Discussion</title><p> Discussion. Studies have shown that vulvar reconstruction using skin flaps can avoid complications and improve patients' quality of life. Reconstruction with flaps is not currently an accepted standard of treatment for vulvar cancer.</p></sec><sec><title>Conclusion</title><p> Conclusion. The methods of the wound defect closure are possible after radical vulvectomy in patients with squamous cell vulvar cancer and resection of adjacent anatomical structures without reduction of surgical treatment volume.
