Ишемическая болезнь сердца — самая распространенная причина смертности в РФ и мире. Одним из основных методов хирургического лечения является эндоваскулярное стентирование коронарных артерий. Несмотря на малоинвазивность метода, существует потребность в обеспечении седации пациента во время вмешательства. Основным видом анестезиологического пособия для данной процедуры является мониторируемая седация.</p><p>Цель исследования — провести сравнительный анализ двух наиболее часто используемых схем мониторируемой седации при плановых эндоваскулярных вмешательствах в мировой практике.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С января по июль 2021 года проведено проспективное когортное исследование, включившее в себя 70 пациентов. Проведено сравнение группы седации с использованием феназепама (n=38) с группой аналгоседации с использованием комбинации сибазона и фентанила (n=32). Сравнение проведено по уровню седации (шкала RASS), степени выраженности ангинозных болей на этапе стентирования коронарных артерий по ЦРШ; витальных показателей пациентов (сАД, дАД, срАД, ЧСС, SpO2).</p></sec><sec><title>Результаты</title><p>Результаты.. Применение схемы мониторируемой седации (сибазон + фентанил) приводит к более глубокой седации по RASS и более значимому снижению частоты сердечных сокращений у пациентов на этапе стентирования коронарных артерий в сравнении с использованием только бензодиазепина (феназепам). Несмотря на это, частота развития ангинозных болей во время вмешательства статистически значимо не различается между группами сравнения.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. В настоящей работе мы сравнили два подхода к анестезиологической защите при выполнении плановых эндоваскулярных стентирований коронарных артерий, которые наиболее часто используются в зарубежной и отечественной интервенционной практике.</p></sec><sec><title>Выводы</title><p>Выводы. Применение аналгоседации (сибазон + фентанил) при плановых эндоваскулярных стентированиях коронарных артерий не имеет существенных преимуществ перед седацией бензодиазепинами (фенозепам) при данных типах вмешательств. Становится очевидным необходимость продолжения поиска более эффективных и безопасных схем анестезиологического обеспечения при плановых эндоваскулярных стентирований коронарных артерий.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Ischemic heart disease is the most common cause of death in the Russian Federation and in the world. One of the main methods of surgical treatment is endovascular stenting of the coronary arteries. Despite the minimally invasive method, there is a need to ensure sedation of the patient during the intervention. The main type of anesthetic aid for this procedure is monitored sedation.</p><p>The aim of the study was to conduct a comparative analysis of the two most frequently used regimens of monitored sedation for elective endovascular interventions in world practice.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From January to July 2021, a prospective cohort study was conducted that included 70 patients. A comparison was made between the sedation group using phenazepam (n = 38) with the analgosedation group (n = 32) — a combination of sibazone and fentanyl. The comparison was made by the level of sedation (RASS scale), the severity of anginal pain at the stage of stenting of the coronary arteries according to the VAS; vital indicators of patients (sBP, dBP, avgBP, HR, SpO2).</p></sec><sec><title>Results</title><p>Results. The use of a monitored sedation regimen (sibazone + fentanyl) leads to deeper sedation according to the RASS and a more significant decrease in heart rate in patients at the stage of coronary artery stenting compared with the use of benzodiazepine (phenazepam) alone. Despite this, the incidence of anginal pain during the intervention did not differ statistically significantly between the comparison groups.</p></sec><sec><title>Discussion</title><p>Discussion. In this work, we compared two approaches to anesthetic protection when performing planned endovascular stenting of coronary arteries, which are most often used in foreign and domestic interventional practice.</p></sec><sec><title>Conclusions</title><p>Conclusions. The use of analgosedation (sibazone + fentanyl) for elective endovascular stenting of coronary arteries has no significant advantages over sedation with benzodiazepines (phenozepam) for these types of interventions.
