Трудности дифференциальной диагностики язвенной болезни и злокачественных новообразований желудка

Введение. Рак желудка представляет собой серьезную проблему современной медицины и является одним из самых распространенных злокачественных заболеваний в мире, находясь на четвертом месте среди причин смертности от онкологических заболеваний. Большинство пациентов обращается за медицинской помощью на поздних стадиях заболевания, как следствие, летальность в течение первого года после установления диагноза составляет около 50 %.

Описание. Представлен случай клинического наблюдения пациента с диагностированной перстневидноклеточной карциномой желудка. В ходе описания выделены ключевые аспекты, требующие внимания при определении факторов риска, тактики обследования и ведения пациента с подозрением на злокачественное заболевание желудка.

Обсуждение. Клиницисту важно помнить, что инфильтративно-язвенные формы рака характеризуются стертой клинической картиной и нередко замаскированы под сопутствующей патологией. Дополнительную сложность представляет проведение дифференциальной диагностики инфильтративной формы рака желудка и язвенной болезни. Только комплексное обследование, включающее в себя клинические, лабораторные, инструментальные исследования, в т. ч. проведение морфологической оценки биоптатов из краев язвы, имеет решающее значение в своевременном установлении диагноза злокачественного заболевания желудка.

Выводы. Обнаружение заболевания на начальных стадиях позволяет не только значительно повысить шансы пациента на полное излечение, но и проводить терапию с помощью максимально щадящих методов. Проведение дифференциальной диагностики между язвенной болезнью и инфильтративным раком желудка представляет особую сложность. Только комплексное клиническое, лабораторное и инструментальное обследования пациентов способствует раннему установлению верного диагноза и, как следствие, своевременному проведенному хирургическому лечению.
