Оценка влияния групповых занятий для беременных женщин по грудному вскармливанию на успешность становления лактации

Введение. С 2026 г. в рамках Программы государственных гарантий в медицинских организациях внедряются школы здоровья для беременных по грудному вскармливанию, что требует разработки и апробации эффективных образовательных программ.

Цель — оценить влияние обучения в школе здоровья для беременных по грудному вскармливанию на успешность становления лактации в раннем постнатальном периоде.

Материалы и методы. Проведено проспективное когортное исследование с участием 228 пар «мать — ребенок». Основную группу (n = 110) составили женщины, прошедшие обучение в школе здоровья на сроке 37–39 недель гестации; контрольную (n = 118) — женщины, не проходившие обучение. Эффективность оценивалась с помощью шкалы LATCH в два временных периода: в первые 6–12 часов и через 72 часа после родов. Статистический анализ проводился с использованием U-критерия Манна — Уитни и χ²-критерия.

Результаты. Медиана суммарного балла по шкале LATCH (Me [Q1; Q3]) в основной группе достоверно выше, чем в контроле, как в первые 6–12 часов (7,0 [6,0; 9,0] против 5,5 [5,0; 7,0], p < 0,05), так и через 72 часа после родов (8,5 [6,0; 10,0] и 7,0 [6,0; 8,0], p < 0,05). В основной группе, по сравнению с контрольной, реже регистрировались проблемы с захватом груди (26,8 % и 38,4 %, p < 0,05), дискомфорт при кормлении (22,6 % и 38,1 %, p < 0,05), а также потребность в дополнительной помощи персонала (22,4 % и 44,2 %, p < 0,05).

Заключение. Обучение в школе здоровья для беременных по грудному вскармливанию способствует успешному становлению лактации.
