The application of macrolids in the treatment of purulent nonrocrical complications of the diabetic stop syndrome

Abstract

Here are the results of a prospective comparative randomized clinical, microbiological and cytological smears-imprint of the wounds of 65 patients with a neuropathic and neuroischemic (without critical ischemia) form of the diabetic foot syndrome complicated by the development of purulent necrotic process, prior to the treatment, on the 4th, 7th, 14th, 14th and on the 21st day of the treatment. The aim of the work is to increase the effectiveness of the therapy to the patients with purulent-necrotic complications of VDS by including a complex antibacterial therapy with a group of macrolides in the scheme. Two groups of patients were examined: 1st group - after the surgical treatment of wounds and intravenous introduction of antibacterial drugs selected according to antibioticogram results; 2 nd - after the surgical treatment of wounds and antibacterial therapy combined (drug, selected according to antibioticogram + clarithromycin).Based on the results of the cytological analysis of smears-prints and the dynamics of bacteriological landscape of the wound surface, the effectiveness of the complex antibacterial therapy with the inclusion of a group of macrolides is shown.At the same time, there was a reduction in both the inflammatory and reparative phases of the wound process, which made it possible to resort to plastic closure of wound defects at an earlier time. There were no complications in performing dermatomic autodermoplasty in the patients of the main group.
