Abstract
Purpose. Identification of characteristics, not included in the CHA2DS2-VASc scale, affecting the risk of ischemic stroke in patients with atrial fibrillation (AF) and one non-sex-related risk factor (RF), due to arterial hypertension (AH), of thromboembolic events on this scale. Methods. A “case-control” study was conducted according to retrospective analysis of medical records of patients with AF. The main group included hospitalized due to development of ischemic stroke patients with non-valvular AF and single additional to gender RF on the CHA2DS2-VASc scale due to AH in the period prior to the stroke development. As a comparison group, patients with comparable demographic characteristics, having non-valvular AF with single additional to gender RF on the CHA2DS2-VASc scale due to AH and no history of stroke or systemic embolism, were selected. Anamnesis data and the results of electrocardiography, echocardiography and ultrasonic duplex scanning of brachiocephalic arteries were analyzed. Results. The main group included 37 patients, age 60.15 (±6,21) years, 27% women. The control group included 88 patients, age 60.13 (±4,85) years, 37.58% women. Patients of the main group, compared to the control group patients, significantly more often showed permanent AF (48.7% and 14.3%, p = 0.01), signs of left ventricular hypertrophy (LVH) according to echocardiography (67.6% and 43.6% , p = 0.026), and thickening of the intima-media complex (cIMT) > 0.9 mm (73% and 25%, p <0.0001) according to ultrasonic duplex brachiocephalic arteries scanning. The univariate analysis showed a significant increase of ischemic stroke risk in the presence of a permanent AF (odds ratio (OR) 5.28; 95% confidence interval (CI) 1,15 - 6,94), LVH according to echocardiography (OR 2.81, 95% CI 1.15 - 6.91) and cIMT thickening (OR 7.88, 95% CI 2.96 - 20.94). Conclusion. When assessing the risk of ischemic stroke and making the decision on the oral anticoagulants appointment in patients with atrial fibrillation and single additional to gender risk factor on the CHA2DS2-VASc scale due to AH, in which the indications for anticoagulant therapy are not absolute, it is advisable to consider the data of echocardiography, ultrasonic duplex scanning of brachiocephalic arteries and the form of AF.

Abstract
Hypoxic-ischemic encephalopathy (HIE) is one of the most actual problems of рediatrics because of the high mortality and disability of children with this pathology. The most promising technique with a high degree of conclusiveness, that is able to reduce the negative effects of HIE, is considered a therapeutic hypothermia (TH). In this article we consider a clinical case of low-tech total TH carried out of newborn with severe HIE. The application of this approach stimulated early recovery of spontaneous breathing, relief of spasms, and also had a favorable impact on the development of the neurological status of the child.

Abstract
Studying of the dental status of the employees of mining and processing works occupied with production and processing of copper-zinc ores is carried out. Objects of a research were 135 workers of Uchalinsky Mining and Processing Integrated Works occupied with production and processing of copper-zinc ores. At workers we have revealed high prevalence of caries of teeth, diseases of fabrics of the parodont and a mucous membrane of an oral cavity. In this regard there was a need of development of methods of treatment and prevention of dental diseases at workers of UGOK. We have created the program of prevention and treatment of dental diseases at employees of mining and processing works. Methods of treatment and prevention can be used for optimization of the existing system of protection of dental health of workers given and other similar enterprises.

Abstract
For the purpose of studying of local immunity of an oral cavity, studying of the dental status of employees of Uchalinsky Mining and Processing Integrated Works is carried out. Objects of a research were 235 male employees of Uchalinsky Mining and Processing Integrated Works aged from 18 up to 60 years and 90 people of control group who don't have professional harm and similar on age and sex sign. We have revealed the return correlation communication of indicators of local immunity of an oral cavity with severity of diseases of the parodont and with an experience of workers. Calculation of coefficient of balance of factors of local immunity at which borderline of local immunity has come to light at miners of UGOK has been made.

Abstract
The aim of the study was the selection of indications for surgical treatment of patients with osteochondrosis with instability using implants disc and interspinous clamps, which have become the clinical case of permanent vertebral and radicular pain syndromes, rentenological confirmed postnukleotomic instability in the vertebral motor various stages of segments, with or without diskoradiculyar conflicts with ineffective conservative treatment (medication and physiotherapy). In turn, the reduction of mineral density in bone tissue of vertebrae at more than 20% (T-scale - and less than 0.8 SD) from peak bone mass, the radiological signs of fibrosis of vertebral disks, and (or) a segment spondylosis, the spondylarthrosis in unstable lumbar segment with productive hyperplastic compensatory component in the capsule of the joints, restricting movement in the joints and in the spinal motor segments, became the contraindications.

Abstract
The results of the evaluation of the effectiveness of the application of therapeutic and rehabilitation complexes based on transendoscopic low-intensity laser irradiation in combination with ozone applications in 56 patients with peptic ulcer disease are presented. It has been established that low-intensity laser therapy with the transendoscopic method and ozone applications against the background of antiulcer drug therapy promotes acceleration of healing of ulcerative erosive processes, leads to the restoration of immunological reactivity with the regress of clinical and functional disorders and improvement of the quality parameters of patients' life.

Abstract
The article presents the results of chemiluminescence of oral fluid in 155 patients of young age (from 19 to 25 years) with chronic gastroduodenitis. Depending on the duration of chronic gastroduodenitis, changes in the luminous sum of radiation and its maximum value in these patients were revealed. The relationship between the chemiluminescence of the oral fluid and the degree of inflammation of the gastroduodenal zone was determined.

Abstract
A high level of cardiovascular diseases and the increase in the number of patients with excessive body weight indicate eating disorders among the population not only of Russia, but also in the whole world. Rhetorical remains the question of determining the primary pathogenetic link. However, the understanding and control of cardiovascular risk will entail improving the quality of life of patients with multiple organ pathology and positive changes in the dynamics of statistical indicators of health. The aim of our study was to study the features of cardiovascular system in 20 patients with nonalcoholic fatty liver disease (NAFLD) and gastroesophageal reflux disease (GERD) aged 18 to 60 years. As a result, we demonstrated a high prevalence of modifiable and non-modifiable factors of cardiovascular risk in patients with NAFLD and GERD, the leading of which was an inherited family history, arterial hypertension, inadequate physical activity, nicotine dependence, excessive alcohol consumption.

Abstract
The aim of the study was to study the effectiveness of our medical and social prevention among patients with chronic pyelonephritis. In order to study the effectiveness, a school was organized for patients with chronic pyelonephritis. The observation covered 138 patients with chronic pyelonephritis, which were observed in school for 2 years. Of these, 67 were the main group, and 71 were the control group. In the main group, we carried out medical and social measures developed by us to eliminate (weaken) the adverse factors that we identified, these measures were not extended to the control group. The use of the medico-social prophylaxis developed by us to eliminate (weaken) the influence of unfavorable social-hygienic and medical-biological factors will reduce the frequency of exacerbations of chronic pyelonephritis, and also prevent an unfavorable outcome.

Abstract
Over the last few decades, a number of studies have shown correlations between arterial hypertension (AH) and the risk of development of malignant neoplasms. A literature review has revealed some associations between the factors involved in AH pathogenesis and malignant growth in kidneys. The effect of the components of the renin-angiotensin-aldosterone system (RAAS) on pathological neoangiogenesis, proliferation and apoptosis has been noted. Treatment of patients with RAAS inhibitors reduces the cancer progression and improves the survival of patients with primary and metastatic kidney cancer. It allows us to assume the pathogenetic significance of the RAAS links for the kidney cancer development and progression. In this regard, the possibility of clinical use of ACE inhibitors and/ or ARBs for cancer patients, including those without arterial hypertension, is being discussed, which opens the prospect of studying their potentially new applications in modern pharmacotherapy not only of hypertension but also of malignant diseases.

Abstract
In order to assess the incidence and nature of violations of penile hemodynamics in the genesis of erectile dysfunction in patients with multiple sclerosis, data on pharmacodopplerometry of penile vessels in 151 men divided into 3 groups (patients with multiple sclerosis divided into 2 groups by severity and control group of healthy juniper) we are studied. On the basis of ultrasound dopplerography and spectral analysis, a significant decrease in blood flow along the arteries of the penis was found in patients with severe neurological disorders in 22.5% of cases with preserved functional capabilities of cavernous bodies. With a slight degree of neurological deficit in most patients, disorders of arterial blood supply and functional ability of cavernous bodies were not detected.

Abstract
The article presents the results of a clinical study of a comparative assessment of the therapeutic efficacy, safety and tolerability of the drug Panavir, a solution for intravenous administration of 0.04 mg / ml with the preparations of the basic complex therapy of IVI in pregnant women in the II and III trimesters. The study revealed that the inclusion of the drug "Panavir" in the complex therapy of viral infections during pregnancy effectively reduces the risk of developing amnionitis, placenta and infection of the fetus; is safe for both the mother and the fetus and is not accompanied by side effects and undesirable phenomena.

Abstract
In recent time the tendency to increase of a frequency of extragenital deseases among pregnant women ,that is associated with the deterioration of the general health, is observed. A comparative analysis of hospital morbidity in pregnant woman living in Dagestan in 15-49 years old with extragenital diseases was conducted. A proportion of women with extragenital pathology among hospitalized women is 25.4%. A share of blood diseases in the structure of extragenital diseases is 52%, diseases of the digestive system - 14.5%, especially among women of active reproductive age (20-29 years old). Therefore, the extragenital patology can cause a development of various obstetric complications and contributes to the deterioration of women's reproductive health and a violation of the reproduction’s processes.

Abstract
To determine the immune status in patients with organ-sparing operations after cesarean section (peritonitis) we examined 30 patients who were divided into 2 groups. The first group consisted of 15 patients with peritonitis after cesarean section who underwent complex treatment of peritonitis was prescribed the antibiotic sensitivity of bacterial flora and radical surgery (amputation or hysterectomy). The second group - 15 patients who underwent complex therapy of peritonitis and the additional laser irradiation of the inguinal lymph nodes on 68-128 seconds. Patients in this group carried out organ-saving operations. All was guided by the following immunological parameters: T-, B-lymphocytes and immunoglobulins M, G, A.

Abstract
Climacteric syndrome remains an urgent problem today. The climax is both the norm and disease: the norm because the menopause in the female body is a natural phenomenon, but a disease because it is a persistent violation of the regulation, leading eventually to decreased viability of the organism. We examined 207 women aged 40-55 years with climacteric syndrome in early natural (110 women) and surgical postmenopausal (97 women). The study of clinical data revealed some differences in the occurrence of menopausal women in early natural and surgical postmenopausal women.

Abstract
Currently, the frequency of genital prolapse in the structure of common gynecological diseases has no tendency to decrease and may take up to 15% of large gynecological operations. With the aim of improving outcomes of surgical treatment of this category of patients, at the stage of preoperative preparation and the postoperative period used vaginal capsules containing Lactobacillus + progesterone 2 mg + estriol 0.2 mg (Triozhinal). The study included 239 patients in postmenopausal women with genital prolapse and phenomena cystourethroscopy and vaginal atrophy. Of these, the main group consisted of 122 women who underwent the proposed scheme of preoperative preparation and postoperative period. Group comparisons were presented 117 patients admitted to the gynecological Department for surgical treatment of prolapse with traditional preoperative and postoperative management. The results of the study showed that in the group of patients receiving local therapy estriola in combination with progesterone and lactobacilli, was observed a significant improvement of the clinical picture of urinary disorders, quality of life, symptoms of urogenital atrophy during surgical correction of urinary incontinence or genital prolapse, and this tactic can be recommended as in the pre-and postoperative periods.

Abstract
The study of diagnostic importance of biochemical screening of pregnant women on AFP and hCG for detection of congenital hereditary diseases was conducted. The main group consisted of 109 patients, in who congenital malformations and/or chromosomal anomalies of fetus or infant were diagnosed prenatally or after the birth. The control group consisted of 280 patients of RPC, whose pregnancy ended with the birth of children without congenital heredity diseases. The sufficiently high efficiency of detection in serum of blood AFP and hCG in the 2nd trimester of pregnancy during the formation of a risk group for congenital malformations (CM) and the chromosomal anomalies is shown in the article.

Abstract
According to foreign authors in the structure of child morbidity and mortality in the majority of developed countries, the congenital malformations of fetus and newborn take the first place [6,7,9,10]. The comparative analysis of macro- and microscopic features of placenta in women with congenital malformations of fetus and newborn (the main group - 37 patients) and in women of comparison group (15 patients) with period of gestation from 35 weeks is shown in this article. Organometric and macroscopic analysis of afterbirth in mothers with congenital malformations of fetus revealed some peculiarities: the anomalies of attachment of the umbilical cord, the disturbances of blood circulation in placenta. During the analysis of signs of infectious pathology, the fact that in mothers of the main group the placental chorioamnionitis occurred more frequent (in 2,5 times), the parietal chorioamnionitis (in 2,12 times), the basal deciduitis (in 1,21 times). The lesion of the intervillous lacuna (the intervillositis) occurred in 1,23 times frequent in the main group than in the comparison group, and the inflammation in the transition of fetus’ membranes to chorionic and basal plates of the afterbirth (marginal placentitis) occurred more frequent in 1,36 times.

Abstract
The congenital malformations are heterogenic group of pathological conditions, including isolated, systemic and multiple anomalies of organs and systems of different etiology: genetic, environmental and multifactorial. [9,11,13,18,23,25,30,32,33, 36,37,38] The majority of congenital malformations have a multifactorial etiology that implies simultaneous influence of genetic predisposition and additional external influences. That is why, the evaluation of prevalence of congenital malformations and their nosological spectrum and etiological factors is one of the modern problems of medical genetics. In this regard, their prenatal diagnosis has a particular importance, one of this main direction is the development of methods to detect the congenital anomalies and hereditary diseases of fetus on early stages of pregnancy. The increase of the quality of ultrasound scanners, the opportunity of digital processing of image, and especially integration in practice the high-resolution transvaginal echography allows to postpone the solution of a number of issues of prenatal detection of congenital and hereditary pathology on the I trimester of pregnancy. [13,14,19,20,21,23,30,36,37]. In this article the assessment of opportunity of ultrasound diagnosis of congenital malformations of fetus during the first trimester of pregnancy was conducted.

Abstract
The article presents an analysis of the official statistical data on the morbidity of preeclampsia and eclampsia for the period from 2006 to 2016 in the Perm region in comparison with the data of the Russian Federation and the Volga Federal district. The obtained results show a multidirectional tendency of morbidity in different regions of the Russian Federation according to data of outpatient and inpatient departments. Thus, according to data of maternity hospitals in the Russian Federation in 2016 the morbidity of preeclampsia and eclampsia was 32.25 per 1,000 births. This morbidity rate remains relatively stable in the Russian Federation and the Volga Federal district for the period from 2007 to 2016. At the same time in the Perm region it has a tendency to increase while in Perm it decreases (decrease 19%). Based on the results of the analysis, the effectiveness of implementation of modern orders and clinical guidelines is shown.

Abstract
Aim. To determine with use of a binary logistic regression the risk of respiratory diseases in children born by caesarean section . Methods. According to the aim of the study the statistical method of binary logistic regression was used. For the study, children born by caesarean section were taken under observation. Results. It was found that the high prognostic significance of the development of the disease in children born by caesarean section was the absence of breastfeeding, low financial position, smoking among parents.

Abstract
Here are the results of a prospective comparative randomized clinical, microbiological and cytological smears-imprint of the wounds of 65 patients with a neuropathic and neuroischemic (without critical ischemia) form of the diabetic foot syndrome complicated by the development of purulent necrotic process, prior to the treatment, on the 4th, 7th, 14th, 14th and on the 21st day of the treatment. The aim of the work is to increase the effectiveness of the therapy to the patients with purulent-necrotic complications of VDS by including a complex antibacterial therapy with a group of macrolides in the scheme. Two groups of patients were examined: 1st group - after the surgical treatment of wounds and intravenous introduction of antibacterial drugs selected according to antibioticogram results; 2 nd - after the surgical treatment of wounds and antibacterial therapy combined (drug, selected according to antibioticogram + clarithromycin).Based on the results of the cytological analysis of smears-prints and the dynamics of bacteriological landscape of the wound surface, the effectiveness of the complex antibacterial therapy with the inclusion of a group of macrolides is shown.At the same time, there was a reduction in both the inflammatory and reparative phases of the wound process, which made it possible to resort to plastic closure of wound defects at an earlier time. There were no complications in performing dermatomic autodermoplasty in the patients of the main group.

Abstract
Optimal drinking regime influences on the plasma glucose level in patients with insulin-nondependent type 2 diabetes mellitus with fixed water consumption deficiency during the diet maintaining and oral antihyperglycemic therapy has been assessed. 103 people: 21 men (20,4%) and 82 women (79,6%) took part in the research. The average age of the group was 64,7±3,2 (men - 61,1±1,01, women - 68,3±5,4). The research for each patient continued for 10 days, in the first 5 days we observed for the volume of the drunk water. If the deficiency of drunk water was found out the patients were offered to the lacking volume with warm boiled water. The deficiency of water for each patient was calculated by this formula: «42,9 (ml) Х weight (kg) - the average patient’s volume of drunk water for 5 days (ml)». The deficiency of the volume of drunk water was 2101,3 in average R (range) [1902,9; 3119,3] ml. The optimal water regime in the group of patients with insulin-nondependent type 2 diabetes mellitus led to depression of the average glucose level of blood plasma. Also it helped to stabilize the glucose level during a day.

Abstract
The purpose of this study was to value efficiency kinesio taping by Kase et al in patients with shoulder impingement syndrome with support treatment. Patients (n = 100) from 30 to 69 years was divided for 2 groups by 50 patients each. The main goal of this study was to measure mobility arms, level of painful feeling and mental condition. Response to treatment was evaluated with the the Disability of Arm, Shoulder, and Hand (DASH) scale, visual analogue scale (VAS), SF-36 BP, SF-36 MH. Measures were assessed before treatment, after treatment and after 1 month. The DASH score of the kinesio taping group was statistically significantly lower after 1 week and 1 month as compared with the control group. So, Kinesio tape has been found to be more effective than the only standard therapy.

Abstract
Nowadays quality of life is becoming a growing concern, including rheumatology. However, there are a few studies on quality of life of patients with Takayasu arteritis. In the light of the above, investigations related to quality of life are rather relevant in case of Takayasu disease. Objective. To assess the influence of comorbidity on quality of life, anxiety and adherence of treatment in patients suffering from nonspecific aortoarteritis Materials and methods. The study included 38 patients different sex and age, suffering from NAA, hospitalized in Ural Regional Hospital № 1 between 2013 and 2017. The design of the research is one-time cross-sectional study. For examine the quality of life of patients with TA we used the Medical Outcomes Study Short Form (SF-36), Hospital Anxiety and Depression Scale (HADS) , Morisky Green Levine Medication Adherence Scale (MGLS) Results: Comorbidity influences on quality of life, adherence of treatment in patients suffering from nonspecific aortoarteritis.

Abstract
Thе aim of this research was acomparative assessment of the herd immunity to epidemic and seasonal influenza virus strains in the epidemic periods of 2015-2016 and 2016-2017 in compliance with the results of a sampling research of the donated blood sera in Yekaterinburg and Sverdlovsk Oblast, the Russian Federation. Hemagglutination inhibition assay test was used to assess 817 samples for antibodies to A/ California/07/09(HlNlpdm09), A/HongKong/4801/14(H3N2) and В/Phuket/3073/13 influenza viruses. Index >1:40was considered a positive antibody titre. In the pre-epidemic period of 2015, the proportion of serapositive to A/California/07/09(HlNlpdm09) virus was 36.4%. In the pre-epidemic period of 2017, this proportion for A/Hong Kong/4801/14(H3N2) virus was 38,0%, in 2017 this proportion for A/California/07/09(HlNlpdm09) virus was 58,7%. The epidemic increases in the incidence caused by the corresponding current influenza virus serotypes in these years imply that prior to every epidemic season the herd immunity of the population was insufficient for the prevention of epidemics.

Abstract
This article presents results of the analysis of the use of herbal medicines among the population of the Kyrgyz republic. A sociological survey was conducted by the method of a sample anonymous survey of the visitors of pharmacies in the 2 largest cities of the Kyrgyzstan: Bishkek and Osh. The survey was conducted with questionnaire which was worked out by us and contained 19 questions of open and closed types with simple alternative and multiple options for answers. 638 respondents’ questionnaire, 319 of them were from Bishkek city and 319 from Osh city, were used for the analysis. We have found out a gender, sociological status of respondents and monthly income per one family member. We have also found how chronic diseases affect the respondent and if they use the herbal medicines for the treatment or prevention of their disease. Also we have established information sources by choosing herbal medicines and how much money spends consumers on the purchase for herbal medicines.

Abstract
This article presents the results of the marketing analysis of the pharmaceutical market for herbal medicines in the Kyrgyz Republic. The materials of the study were the database of the Department of Medicine Supply and Medical Equipment under the Ministry of Health of the Kyrgyz Republic and the State Register of Medicines and Medical Products for 2010-2015. Also, a classification, statistical, comparative and economic analysis of the use of phytopreparations was conducted. We analyzed the dynamics of registration of phytopreparations of domestic and foreign production by classifying them according to the forms of release and the types of therapeutic effect. We also determined the amount of phytomedicines delivered to the market of the Kyrgyz Republic in natural terms for each year from 2010-2015. In addition, we compared the growth rate of phytomedicines market in the Kyrgyz Republic with the average price of packaging of herbal products for 5 years and identified the main countries that supply phytopreparations for the pharmaceutical market in Kyrgyzstan.

In animals with intermediate resistance to hypoxia, there was initially a higher level of corticosterone than in intact animals and in animals with both high resistance and low resistance to hypoxia. Animals with intermediate resistance to hypoxia were characterized by greater endurance during swimming at a low temperature.

Abstract
The article presents an analysis of premature mortality of working-age population from diseases of the digestive system and the assessment of related economic damage in the Sverdlovsk region. Mortality analysis was performed over the period 2003-2016using official statistical information and the database of the regional Ministry of Health. The mortality from diseases of the digestive system increased, among the working age population by 24.6% in 2013-2016, exceeding the average indicators in Russia. In the structure of the mortality causes dominate liver fibrosis and cirrhosis (43.5%), alcoholic liver disease (22.8%), pancreatic disease (12.8%). The highest mortality rate is among men, from fibrosis and cirrhosis in the age group 35-39, from alcoholic liver disease - in 40-44, from diseases of the pancreas - in 50-54. The annual economic loss due to premature mortality in the working age from diseases of the digestive system exceeds 600 million rubles.
